Treasure Chest feedback form

Organisations receiving KM Treasure Chest prizes are required to complete the feedback form below. Your original offer letter will have included an Application Reference Number which should be added to the feedback form.

Please upload a copy of any posters or newsletters used to promote the event that include the logos, advert or make reference to Treasure Chest or supporting leisure attractions.

If you are unable to scan and upload your documents you should post these with a note clearly stating the name of organisation and name and date of the event.
